Total Knee Arthroplasty (TKA): Step-by-Step Surgical Photo Tour • Step 23 of 24

Surgical Step Description
Following watertight medial retinacular repair with interrupted heavy braided sutures, the knee is allowed to flex freely under gravity alone to guarantee that unconstrained post-operative flexion is preserved.
Master Class Pro-Tip: Intraoperative Pearl
Perform the gravity flexion test immediately after capsular closure: when the thigh is supported and the leg released, the knee should bend under gravity alone to at least 90°; failure to do so indicates excessive capsular overtensioning requiring stitch readjustment.
Master Class Pro-Tip: Gravity Drop Test & Capsular Tension Verification
