Question
A 55-year-old man has pain, swelling, and decreased motion in his right knee after undergoing primary total knee arthroplasty 6 months ago. Laboratory studies show an erythrocyte sedimentation rate of 59 mm/h (normal up to 20 mm/h) and a C-reactive protein level of 15.4 (normal less than 10). Aspiration of the knee reveals a WBC count of 40,000/mm3 (normal 3,500 to 10,500/mm3), and the bacterial culture is negative.
Management should now consist of
1- anti-inflammatory drugs, ice, and aggressive physical therapy.
2- sympathetic blockage.
3- oral antibiotics.
4- surgical debridement and polyethylene exchange.
