Answer (click for answer)
This patient has an increased TMT joint angle as well as an incongruent MP joint (the articular surface of the MT is in anatomic alignment, the P1 is subluxed and angulated laterally. X-ray reveals a hallux valgus deformity with an Intermetatarsal angle of approximately 20 degrees (nl = 9) and a non-congruent MTPJ with a hallux valgus angle of approximately 40 degrees. A distal procedure alone would not correct the proximal deformity. A distal osteotomy wound not be indicated because the distal articular surface is in normal anatomical alignment. In this case a proximal osteotomy is needed to realign the 1st MT and decrease the TMT angle. A distal soft tissue release would also be needed to realign the phalanx over the metatarsal and decrease the hallux valgus angle.
